A record is only worth what it can prove

A number in a report is a claim. What makes it evidence is the chain behind it: the source it came from, the transformation applied to it, the person who approved it, and the timestamp on each of those.

When that chain is missing, the number is not wrong — it is unusable. Whoever has to rely on it has no way to check it, so they ask for it again, by hand, from the people who were there. That request is where most of the delay in a regulated process actually lives.

Append-only, or it is not a trail

An audit trail that can be edited is a report. We build the record so entries are added and never overwritten: a correction is a new entry that supersedes an earlier one, and both remain visible with the reason for the change.

Every entry carries the same fields — who, when, which source document or photo, what changed, who approved it. That uniformity is what lets a reviewer answer a question about any record without knowing which system produced it.

Built into the workflow, not bolted on afterwards

Evidence collected after the fact is reconstruction, and reconstruction is where disputes start. So the trail is written at the moment the work happens — at capture, at extraction, at validation, at the review queue, at approval.

Nothing writes to the record without passing through the same path, including the agents. An automated step is logged the way a human step is, with the input it saw and the rule it applied.

What it is for

Field work that has to become a document a lender will accept before a drawdown is released. Compliance reporting that has to be defensible months after the person who filed it has moved on. Any process where the question is not what happened but how you know.

The output is a record that answers that question on its own, without a person reassembling it from email.
